Immunization Requirements

Iowa Code 139A.8 requires all students enrolling in an Iowa school to submit proof of immunizations to the admitting official using an Iowa Department of Public Health certificate of Immunization or a provisional certificate, unless a valid certificate of religious or medical exemption has been submitted.

Please note that there is a new requirement that all students entering the 7th grade are required to have a booster of Tdap (tetanus, diphtheria and pertussis) this is an action taken to try to control the pertussis or whooping cough. If you have an incoming 7th grader please turn in a record of the booster. A great time to get this done is when you have the athletic physical completed.

All students entering preschool and kindergarten must have a valid certificate of immunization. Please turn this in with the required physical for entrance into school.

Medication administration:

If you have a student who requires a prescription medication during the school day please print the medication administration paperwork off the e-registration tab on the schools web site and have your health care provider fill it out.
