Health and Fitness : Community Health Needs Assessment

Virginia Gay Hospital in collaboration with Benton County Public Health to conduct a survey assessing the top health and community needs of the people we serve. The following survey questions are based on six factors important for the health and well-being of everyone in your community. We are interested in your input.The survey will take approximately 15 minutes to complete. Results of this survey will be available later in 2022. All responses will be anonymous and confidential.
